Jotform is great! I have used it for several automation items – for example, team enrollment for insurance (online) and check requests for reimbursement for the church. You can generate HTML code for it and embed the code in your web site easily. It works well and dependably, and it automatically sends emails reminding of deadlines and/or giving notice of new forms submitted. It offers great programming tools and is easy to set up. Overall my satisfaction is very high with it. One caution…the free part is only for a limited amount of forms and input. It works for me because I only need a few forms.
Google Forms can be sent to an unlimited number of recipients, all for free. Our mission organization has successfully used Google Forms for many years.
I have not used Jotform, but I use Cognitoforms (https://www.cognitoforms.com/) which has very powerful form-creation features. I have been very happy with them. They offer an unlimited number of forms and up to 500 entries per month in their free tier.

This looks great, but you only get 5 forms with the free version. I need a few more for my online courses. So it’s a great idea if you don’t need many forms..but I think I’ll stick w Google Forms or maybe migrate to Microsoft. $19.50 per month for up to 25 forms is too rich for my budget, when I only use them occasionally…but I need more than the 5 freebies. Thanks anyway.
We use the free version, which is adequate most of the time. We found it necessary in an era when Google forms did not support pictures or attachment. We still use it, but has become less necessary for us.
We recently needed to start looking into a team option and JotForm said their cheapest team plan is $7,128 per year. Yikes! We are looking for other options that are good for 2-3 users now and would love to hear about alternatives.
