Elim Bible Institute, Lima, New York has a two year Community Health Program. Very intensive. All courses taught by Medical Professionals. Graduates presently serving or have served in hospitals/clinics in the Phillippines, Kenya, the Democratic Republic of Congo.
This program is offered every other year. Fall of ’09 will begin the next cohort.

I just want to add my vote for Karen’s recommendation. Look into LifeWind International’s CHE strategy at lifewind.org.
I’ve been through the training and have actually become a trainer (though I’m not part of LifeWind). Our partners in Ghana and Peru are using CHE. It’s a very effective strategy for community transformation. 80% of health problems can be prevented or treated at home through CHE lessons, taught by common folks without formal medical training. Check it out!
